    It measn that someone’s got addicted to drugs or alchohol, and has been put in to ‘rehab’ (a rehabilitation centre), and they’ve just been let out.

    It can mean that someone has recently been discharged from a rehabilitation clinic, either after a stroke or accident which required physical training, or after a detoxification and rehabilitation from drug abuse.

    Rehab is short for rehabilitation facility. It’s a place where drug addicts/alcoholics go (or are forced to go) in order to stop drinking or using drugs.

    “Fresh out XX” (XX being any variety of things) means that someone has just left somewhere, and it’s quite informal. I imagine that it comes from “fresh out of the oven” which is used to describe baked foods – usually cakes, muffins, cookies etc. – when served soon after they’ve finished baking (for example:

    Person A: “These cookies are good, when did you make them?”
    Person B: “They’re fresh out of the oven!”)

    When used in other contexts it’s often extremely informal and when it’s used in reference to a place to where someone doesn’t want to/shouldn’t have to go, like rehab or jail, it’s usually used when someone has left that place recently and is already behaving in a way that will send him or her back to there.

    In the context of rehab, it makes the most sense if it’s being used to describe someone who has recently left rehab and has already forgotten the lessons he/she learned there. Another responder gave a good example (I think it was “fresh out of rehab and already drinking again”) – to be honest, pretty much any other example would just be a variation of that one (e.g. “I can’t believe that guy. He’s fresh out of rehab and he’s already getting high”). Fresh out of jail/prison would be basically the same for example: “he’s fresh out of prison and already stealing cards again”
